چکیده
Background and study aim. Pancreatic surgery remains complex, particularly for borderline resectable locally advanced tumours. Vascular invasion compromises resectability, vascular resection entails increased morbidity mortality. Following a feasibility safety demonstration of an augmented endoscopic ultrasound (EUS) guided radiofrequency ablation (RFA) using hydroxyethyl starch (HES) in porcine pancreatic parenchyma, the present assesses whether this approach (EUS-sugar-RFA) perivascular space is safe creates controllable margin necrosis to enable vessel-sparing resection. Methods. EUS-sugar-RFA parenchyma adjacent splenic artery vein was performed live animal model. different survival periods (0-4 days) interventional group (n=3), open pancreatectomy carried out. The control (n=4) included pancreatectomies two pigs with non-treated pancreases RFA alone on same day. Results. All procedures were completed successfully, without intra- or postoperative complications. Survival uncomplicated. Histopathological examination showed local inflammatory reaction at sites. wall integrity preserved all specimens. untreated zones no from normal group. Conclusions. Preoperative HES safe, model best timeframe within 24h prior surgery. This technique might improve resectability selected cancers.

Endoscopic ultrasound-guided radiofrequency ablation of a pancreatic neuroendocrine tumor.
Radiofrequency ablation (RFA), a method of achieving tumor necrosis by cell protein denaturation that is induced by heating tissues to above 45°C, is applied in the treatment of several malignancies. Energy is provided by an RFA current generator, which is connected to an active electrode needle that is placed into the tumor under image guidance. The lesions created have variable diameters, dep...
متن کاملEndoscopic ultrasound-guided ethanol ablation of pancreatic remnant following complicated total pancreatectomy.
Endoscopic ultrasound (EUS)-guided ethanol ablation of pancreatic tissue has been documented in pigs [1,2] and in humans in pancreatic insulinomas [3,4] and in pancreatic cysts [5]. No data are available on the effect of ethanol injection into normal human pancreas, including accidental injection during treatment of pancreatic lesions. A 59-year-old man underwent total pancreatectomy following ...

A 70-year-old woman presented with an 18-month history of recurrent episodes of dizziness that were alleviated by sugary drinks. During one of these episodes, a low blood glucose of 1.6mmol/L was noted. Her past medical history included atrial fibrillation and severe chronic obstructive pulmonary disease (COPD), necessitating long-term oxygen therapy. Given the high index of suspicion of sponta...
متن کاملEndoscopic Ultrasound-Guided Radiofrequency Ablation of the Pancreatic Tumors: A Promising Tool in Management of Pancreatic Tumors
Objective. Radiofrequency ablation is a well-established antitumor treatment and is recognized as one of the least invasive therapeutic modalities for pancreatic neoplasm. Endoscopic ultrasound-guided radiofrequency ablation (EUS-RFA) delivery can be used to treat both pancreatic cancer and asymptomatic premalignant pancreatic neoplasms and may serve as a less invasive alternative to surgical r...
متن کاملEndoscopic Ultrasound-Guided Radiofrequency Ablation (EUS-RFA) of the Pancreas in a Porcine Model
Backgrounds. Limited effective palliative treatments exist for pancreatic cancer which includes surgery or chemotherapy. Radiofrequency ablation (RFA) uses high frequency alternating current to ablate diseased tissue and has been used to treat various tumors. In this study, we evaluated a prototype probe adjusted to the EUS-needle to perform EUS-RFA to permit coagulative necrosis in the pancrea...
